LUBBOCK, TX - Covenant has a program called Patient Navigation and Care Coordination that helps people with chronic diseases.
Teri Pierce is the director of care management for Covenant Health Services. She said this program has been around since 2012.
"Basically what it is, is just to help our patients navigate the complexity of the healthcare system," Pierce said. "My team is out in the community and so our main goal is to help fulfill taking care of the whole person, the mind, body, and spirit."
That team is made up a diverse group of people, to help get the best care for the patients.
"Our program is strictly education based, so we provide disease education, some examples are congestive heart failure, diabetes, hypertension, high blood pressure, heart disease, asthma, so we basically teach people how to take care of themselves and be responsible for their own health," she said.
